**Job Description**

As a Hospice RN, you will be responsible for providing direct patient care services to hospice patients and supportive care to their families. You will work closely with an interdisciplinary team to develop and implement individualized plans of care that meet the unique needs of each patient.

You will also be responsible for:

Evaluating patient and family needs and assisting in the preparation and execution of the plan of care for each patient
Acting as primary liaison between physician, patient/family and hospice team
Empowering patient's caregivers by educating them on care for their loved one
Maintaining all assigned patient clinical records
Participating in company quality assessment and performance improvement programs


**About You**

To be successful in this role, you will need to have:

Hospice heart: compassionate, kind and empathetic to your patients and their families
Comfortable providing care wherever the patient calls "home"
Demonstrated flexibility and composure while supporting others in a time of crisis
RN license in the state of requested employment (or eligibility to obtain)
1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
