The Role:
We are seeking a Registered Nurse to join our team as a Community Nurse. As a Registered Nurse at RSL LifeCare, you will be empowered to make a meaningful difference to people's lives. Within our supportive work environment, your skills and compassion will be valued.

Key Responsibilities:
* Deliver clinical care in the home environment supporting clients to stay living in their home and connected to their community
* Embody a person-centred approach, recognising and promoting the individual's unique goals and preferences
* Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered
* Organise and conduct case conferences and maintain open communications with clients and their families, carers and authorised representatives?* Effectively prioritise client needs in collaboration with other medical and health professionals as required?

About You:
We are looking for a compassionate and empathetic individual who possesses excellent communication skills, both verbal and written.
