Purpose and Scope of the Trusted Assessor Role

The primary objective of the Trusted Assessor is to enhance the quality and efficiency of hospital discharge processes, leveraging technology and inter-agency partnerships to optimize patient outcomes.

Job Responsibilities

As a key member of our team, you will play a crucial role in coordinating proportionate assessments, facilitating effective transfers of care, and contributing to the development of E-Toc documentation.
