This is an exciting opportunity for either an established Band 6 or a Band 5 who wishes to develop with full support. It will allow you to gain confidence, to be a dynamic practitioner, use various models of practice and the chance to be truly holistic within your role.

The post holder provides the professional nursing interface between hospital, CHC Fast Tracks, Discharge to Assess (DtA) and community services, promoting a safe, smooth transfer of care of patient to either their own home, or a residential/nursing home.

To maintain accurate and up-to-date records on all patients, to ensure documentation meets agreed standards. Undertaking key tasks such as Nursing Needs Assessments, CHC Screens, Fast Track Discharges and equipment facilitation.

To identify patients who will require complex packages of care, working closely with the multi- disciplinary team.

Advising, assessing and facilitate the ordering of equipment by the discharge team as appropriate.

Involve the patient, relatives, carers and all agencies involved in the setting up of the care package.

Instigate case conferences as appropriate and attend multi-disciplinary meetings which may require visiting other hospitals.

To make referrals, according to need following discussions with patients and carers to other statutory and voluntary agencies.

To take appropriate action to deal with emergency situations.

To communicate relevant information to patients, families and carers whilst maintaining confidentiality.
