Role Summary:
We are seeking a highly skilled Aged Care Nurse Practitioner to join our team within the Community and Virtual Care Directorate. As an experienced healthcare professional, you will provide expert clinical care to older patients with acute and complex needs. Your responsibilities will include collaborating with medical staff, nursing staff, and other health professionals to deliver high-level clinical services, assessing and managing geriatric patients, investigating medical conditions, prescribing medications, and referring patients to other healthcare professionals as needed.

Key Responsibilities:
Collaborate with medical staff, nursing staff, and other health professionals to deliver high-level clinical services.
Assess and manage geriatric patients using innovative virtually enabled models of care.
Investigate medical conditions and prescribe medications as needed.
Refer patients to other healthcare professionals as required.
Requirements:
Degree-qualified registered nurse or equivalent qualification.
Postgraduate qualifications in advanced practice nursing or equivalent.
Current registration as a nurse practitioner with the Australian Health Practitioner Regulation Agency (AHPRA).
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
