What you'll be doing

The Aboriginal Clinical Nurse Consultant (CNC) is an expert nurse who provides leadership, advice and recommendations on nursing practice, ensuring safe and appropriate care. The role delivers high‑quality, culturally appropriate, person‑centred nursing care within a holistic framework aligned with NSW Health and NSLHD values.

The Aboriginal CNC develops and implements policy, guides practice improvement, contributes to education and research, and provides clinical supervision while acting as a role model in regular clinical practice within Aboriginal health.

The Aboriginal CNC delivers evidence‑based nursing care that supports safe, effective and culturally appropriate services for Aboriginal people with chronic disease.

The role will:

Work within a multidisciplinary Aboriginal health team across NSLHD, supporting community‑based chronic disease services and strengthening links with NSLHD services, GPs, NGOs and Aboriginal Community Controlled Services.
Advocate for clients and families, support informed decision‑making, coordinate care across NSLHD, and ensure follow‑up needs are met, including:
48‑hour follow‑up program
Inpatient visits across NSLHD
Leading community engagement projects and events
Communicating respectfully in line with NSLHD CORE values
Improve transitions from hospital to community care by formalising processes between Aboriginal‑specific services.
Promote Primary Health Care principles using a Social Determinants of Health model.
Candidates will need to meet the following criteria:
Demonstrated knowledge and culturally informed understanding of Aboriginal cultures, histories, and the ongoing impacts of colonisation on health outcomes, with particular expertise in Aboriginal health frameworks and the principles of Social and Emotional Wellbeing.
