JOB SUMMARY
Under the direction of the Director of Case Management, oversees daily operations and supervision of the NCH RN Inpatient and Emergency Department Case Management Team at all NCH campuses. Promotes patient/family focused care as the highest priority and strives to meet customer expectations for quality and patient satisfaction. Utilizes performance management expertise to facilitate the attainment of goals and holds staff accountable to goals, and adherence to policies/protocols.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Other duties may be assigned.
Implements organizational case management strategies with a focus on patient experience.
Ensures compliance with regulatory standards.
Train, and mentor RN case managers.
Completes personnel functions including staffing, performance reviews and corrective action.
Monitors and meets productivity standards.
Implements discipline best practices.
Supports staff by identification and utilization of resources.
Promotes professional development of Case Management Team.
Holds regular staff meetings encouraging staff to share and participate in organizational and department quality improvement.
Ensures staff participation in daily multidisciplinary rounds to discuss transition of care and any barriers to the discharge plan.
Responsible for, but not limited to length of stay, avoidable days, prevention of readmissions, and discharge planning metrics.
Participates in weekly complex case review.
Recording /reporting of indigent fund spending.
Implements appropriate monitoring techniques to ensure case management documentation is completed per department guidelines.
Assures timely, effective discharge planning that involves care team, payor, along with patient and their families.
Advocates and escalates any care delays.
Participates in quality-of-care initiatives; reporting quality concerns to appropriate individuals, departments, and/or committees in a timely manner.
Assists in the development and oversees implementation of care management policies and procedures.
Collaborates with Social Work Manager to ensure effective collaboration with Social Work team.
Collaborates with utilization management team to ensure timely delivery of regulatory forms, appropriate level of care, long stay outliers, and patients exceeding geometric length of stay.
EDUCATION, EXPERIENCE AND QUALIFICATIONS
Minimum of BSN required: MSN preferred.
Minimum of 2 years of supervisory/lead/manager level of experience in hospital Case Management is required
Licensed as a Registered Nurse (RN) in the state of Florida or appropriate compact licensure.
Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association
Case Management Certification preferred.
Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows
