Position Overview
We are seeking a passionate and dedicated Family Case Manager to join our team and work within the Inn’s family shelter. This role focuses on assessing family needs, developing personalized housing-focused case plans, and connecting families with essential resources to support stability and enable them to transition to long-term housing. This role supports using a trauma-informed, client-centered, and strengths-based approach, with services rooted in housing-focused principles. The Family Case Manager plays a critical role in helping families navigate their journey from shelter toward stability, self-sufficiency, and permanent housing.


What You’ll Do

Build a rapport with shelter families through regular meetings to provide support, set goals, and monitor progress toward stable housing.
Offer emotional support and immediate crisis assistance to help families navigate challenges.
Conduct intakes and assessments to identify needs, risks, strengths, and develop short- and long-term goals.
Create and implement personalized case plans focused on achieving permanent housing.
Advocate for families and connect them with culturally relevant resources, including housing, employment, mental health, healthcare, and financial aid.
Maintain ongoing communication with Inn from the Cold teams to ensure wraparound support.
Work with Housing Specialists, service providers, and landlords to facilitate housing searches and transitions.
Support housing stability through programs like tax clinics, budgeting, employment workshops, and ID services.
Complete applications for Income Support, Subsidized Housing, and Fair Entry.
Build and maintain community partnerships; attend relevant community meetings.
Provide warm handoffs as families move into housing or transition to other programs.


This position description reflects the general duties of the role; however, it is not intended to include all duties that may be inherent in the position. The Family Case Manager may be asked to perform other duties as required or assigned.


What You Bring to Our Mission

Knowledge and experience of diverse cultures and worldviews, community, and history.
Completion of a diploma or degree program related to social work, family case management, or human services is preferred.
Requisite sector experience may also be considered in combination with relevant coursework and case management practice knowledge.
Knowledge of trauma-informed and client-centered care, crisis intervention, and conflict resolution skills.
Knowledge and understanding of issues related to homelessness and its impact on families
Ability to respond to the complex needs of families in a family shelter setting.
Ability to plan and organize case plans effectively, monitor progress, and assist the family with goal completion.
Knowledge of housing-focused community resources, services, and supports.
Current First Aid/CPR is required.
Suicide Intervention (ASIST), Self-Harm and Crisis De-escalation training is an asset.
Strong work ethic, sound judgment, and reasoning skills, along with a commitment to discretion and family and staff confidentiality.
Proficient documentation, including case notes and case plans, and survey reporting skills using a client information management system (HOMES).
Computer literacy skills are required, including the ability to use MS Office 365 and Teams.
Excellent communication, problem-solving, and teamwork skills.
Able to communicate effectively both verbally and in writing in the English language.
Police check and Child Intervention check are required for employment.
