About This Opportunity
We are seeking a highly skilled and motivated individual to fill the role of Education Transition Coordinator. As a key member of our team, you will work closely with schools to provide support and guidance to students who are at risk of not successfully transitioning from school before the completion of Year 12. Your key responsibilities will include:

Provide appropriate support and intervention to young people and their families to assist in the transition of young people into a viable education, training or employment option.
Facilitate the development of a school-to-post-school transition plan for young people who leave school before the completion of Year 12 and provide appropriate case management and support as required.
Maintain a reporting framework and report on the progress of identified 'at risk' students to individual school principals and/or their nominee as well as reporting to relevant regional personnel and a Cluster Governance Body. Read more
