Physical Therapist (PT)
Agency: The Ohio State University Wexner Medical Center Home Care
Schedule: Monday–Friday, 8:30 AM–5:00 PM
Territory: Columbus – OSU/Columbus Area
Position Summary
Join a compassionate interdisciplinary home health team providing skilled physical therapy (PT) services to patients in their homes. You'll evaluate, develop, and implement individualized treatment plans to help patients restore function, improve mobility, and reduce hospital readmissions while supporting patients and families throughout their recovery journey.

Qualifications
Current Physical Therapist (PT) license in Ohio
Minimum 1 year of PT experience in an acute care setting
Home health experience preferred
Strong communication and clinical decision-making skills
Valid driver's license and auto insurance
Key Responsibilities
Complete evaluations, OASIS assessments, and individualized care plans
Provide skilled PT services in patients' homes
Educate patients and caregivers on treatment plans and equipment use
Monitor progress and communicate changes with physicians and care team
Supervise PTAs as appropriate
Maintain timely, accurate documentation and regulatory compliance
Requirements
Reliable transportation and ability to travel throughout assigned territory
Ability to lift up to 50 lbs. with or without assistance
