Job Description
We’re looking for an experienced Registered Nurse to lead our Home-Based Support and NASC teams, ensuring people receive high-quality, person-centred care that enables them to live safely and independently in their own homes. In this rewarding leadership role, you’ll provide day-to-day oversight of a multidisciplinary team, drive service excellence, support staff development, and help shape the future of community-based care.
Key Responsibilities
Lead and inspire a multidisciplinary community-based team.
Foster a culture of collaboration, accountability, innovation, and excellence.
Ensure high-quality needs assessment and service coordination practices.
Contribute to multidisciplinary care planning and transitions of care.
Manage staffing and service coverage to meet client needs.
Lead team meetings and contribute to quality improvement initiatives.
Build team capability through coaching and ongoing learning opportunities.
Identify training needs and support workforce development initiatives.
Monitor service performance and contribute to continuous improvement activities.
Champion a strong health, safety, and wellbeing culture.
To be successful in this role, you will have:
Current registration as a Registered Nurse with a valid Annual Practising Certificate
Leadership experience in a healthcare or community setting (preferred)
Excellent communication skills
A relevant postgraduate qualification (or working towards one)
InteRAI Community and/or Palliative Care experience.
Knowledge of NASC processes and community-based support services.
A full and current driver’s licence

Make a meaningful difference for Tamariki/children and their whānau in our Hokianga communities.
We are seeking a compassionate and experienced Community Health Nurse to join our dedicated Primary Healthcare Services Team. This role focuses on promoting and supporting the health and well-being of tamariki/children and their whānau through prevention, education, early intervention, and culturally responsive care.
Working directly within the community, you will act as a vital link between whānau and the wider healthcare system. You’ll deliver high-quality nursing care, provide health education, support early childhood development, and help whānau navigate the services they need.
The ideal candidate will:
Be a Registered Nurse with a current NZ Annual Practicing Certificate, and have at least two years’ post-graduate nursing experience in New Zealand (experience in Māori health settings is highly valued)
Work autonomously in a community setting while contributing positively within a wider multidisciplinary team
Be reliable, flexible, and comfortable navigating a busy, rural healthcare environment
Be committed to empowering whānau to achieve the best possible health outcomes
Have a full NZ Driver’s Licence (essential)

The Role

NURSING OPPORTUNITIES
