Graduate of an accredited Doctor of Pharmacy Program with current licensure as a pharmacist in state of practice
Preferably have completed an ASHP accredited residency program in Oncology or other advanced training/certifications such as Board Certified Oncology Pharmacist
Must have the ability to use the Pharmacy computer system at an acceptable level after having been instructed during the training period
The Lead Oncology Pharmacist must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the disease and age of the patients served on his/her assigned unit
The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to their therapy, and to provide the pharmaceutical care needed

Minimum Requirements:
