Role Description The Health Care Aide, LPN role is a casual remote position focused on providing exceptional home-based care to seniors. Day-to-day responsibilities include supporting clients with activities of daily living such as bathing, dressing, mobility, and toileting, while promoting safety, dignity, and independence. The role involves performing personal care, basic nursing tasks within LPN scope of practice, monitoring client condition, and reporting changes to supervising clinicians or care coordinators. The Health Care Aide, LPN will follow individualized care plans, assist with medication reminders, and provide companionship and emotional support. The position also includes clear documentation of care provided, effective communication with clients, families, and the care team, and adherence to Nurse Next Door policies and regulatory standards.

Qualifications
• Demonstrated experience in home care settings, including in-home support and community-based services.
• Strong communication skills to interact effectively with clients, families, and interdisciplinary care teams.
• Competence in providing activities of daily living support, such as bathing, grooming, mobility assistance, and meal support.
• Solid patient care skills, including monitoring client status, following care plans, and recognizing when to escalate concerns.
• Ability to deliver respectful, person-centered personal care, maintaining client privacy, comfort, and dignity.
• Active LPN license in good standing in the relevant jurisdiction.(For LPN's)
• Current CPR/First Aid certification and knowledge of basic infection prevention practices.
• Reliable internet access and comfort using digital tools for documentation, scheduling, and remote team communication.
• Ability to work independently, manage time effectively, and adapt to changing client needs.
• Experience with senior care, dementia care, or palliative care is an asset.
