Our community nursing services operate seven days a week.
We would welcome applications from anyone who is able to work autonomously in the community including new graduates.
Qualifications & experience
Registered Nurse with current APC, experience in community care or aged care desirable, but new graduates apply.
Exceptional communication skills with the ability to work effectively within an innovative interdisciplinary team,
Intermediate level computer and on-line information recording skills
Strong clinical expertise, excellent assessment skills, desire to learn and grow, and be part of a cohesive and supportive team.
Be able to manage and cope with multiple demands on your time.
Thrive on making a contribution to the delivery of exceptional services and achieving excellent health outcomes for the clients.
Tasks & responsibilities
Working within the Registered / Enrolled Nurse scope of practice, you will engage in direct assessment and clinical care of the people we support.
You would provide on ground support to clients – community – include care planning, wound care, associated documentation, client case review/discharge meetings.
Maintain the well-being of the people we support through effective nursing assessment, diagnosis, planning, intervention and evaluation.
You will provide a broad range of care to clients in their own home such as: a person who is recovering from an injury, completing hands on nursing tasks (such as wound, personal care, catheter and medication management), right through to people who require long term support to remain safe and well in their own home with acc clients, palliative care, dementia care, chronic disease management.
Deliver a high standard clinical service to our clients, along with supervision and support for members of the team.
Self-directed ability to plan and organize assignments effectively, to establish priorities and meet deadlines in a demanding, client focused environment.
