Responsible for the distribution and provision of safe, effective, and timely medication therapy through drug utilization review and collaboration with other healthcare professionals
Responsible for directing any combination of pharmacy technologists, pharmacy learners, or support staff in the mixing, packaging, labeling, and distribution of medications
Interprets orders for medications and ensures therapy is appropriate for patient's diagnosis, weight, concomitant medications, drug allergies and age (pediatric, neonate, adolescent, adult, and geriatric)
Compounds and dispenses prescribed medications per orders in all patient care areas in accordance with departmental and applicable laws
Detects, prevents, evaluates, and reports, when necessary, Adverse Drug Events
Utilizes professional judgment to review medication interactions, contraindications, and disease state precautions, then makes appropriate recommendations to the prescriber to reduce risk and increase quality of medication treatment
Maintains accurate records and patient medication profiles, and develops patient care plans
Provides medication counseling to patients to ensure accurate understanding of treatment purpose, duration, special instructions of use, storage, and common adverse effects
Provides drug information to members of the healthcare team
Recommends and administers vaccines
Participates in quality assurance programs in the Department of Pharmacy
Supervises the activities of supportive personnel and reinforces policies and procedures
Assists in the training and development of departmental staff
Serves as preceptor and/or advisor for pharmacy learners

JOB SUMMARY:
