Clinical & Care Partnering Manager
Job Description
This is a key leadership role within our new Service Region model, focused on delivering high-quality, person-centred care and strong clinical governance across the region.
Your Role
As the Clinical & Care Partnering Manager, you will lead and oversee the delivery of clinical care and care management services in the community, ensuring safe, consistent and high-quality outcomes for clients.
Working closely with the Service Region Manager and multidisciplinary teams, you will provide clinical leadership, strengthen service integration, and support continuous improvement across the region.
Key Responsibilities
Clinical Leadership & Service Delivery
Provide operational leadership for the delivery of evidence-based clinical services and care management
Oversee practice in clinical assessment, care planning and ongoing reviews aligned to client goals and preferences
Support best practice clinical interventions including chronic disease management, wound care and palliative care
Promote a person-centred, coordinated approach to care across multidisciplinary teams
Quality, Risk & Compliance
Ensure services meet Aged Care Quality Standards, legislative requirements and organisational policies
Lead clinical risk management, including incident oversight, investigation and continuous improvement actions
Monitor clinical governance frameworks and implement strategies to enhance safety and quality of care
Conduct audits, reviews and reporting to support compliance and accreditation
People Leadership & Partnering
Provide professional guidance, mentoring and support to clinical, allied health and care management teams
Foster a culture of collaboration, accountability and continuous learning
Build strong relationships with internal stakeholders and external partners to enhance service coordination
Operational Oversight
Excellent communication and stakeholder engagement skills
