Main Responsibilities
Physiotherapist:
To be professionally and legally accountable for all aspects of own work. To manage a patient caseload, minimizing clinical risk and ensuring a high standard of care.
To carry out physiotherapy assessments, treatment planning and implementation to a high standard, evaluating and modifying your input as appropriate.
To communicate effectively with patients and carers, in order to gain informed consent, achieve accurate assessment, to facilitate patient choice and maximize patient engagement.
To communicate effectively with colleagues of all professions and with other agencies to achieve goals, including effective discharges. To ensure that confidentiality standards are maintained.
Occupational therapist:
Provide an evidence-based Occupational Therapy service of a high standard, to patients in a community reablement setting.
Formulate and deliver individual Occupational Therapy programmes based on a sound knowledge of the relevant evidence base and intervention options.
Allocate, supervise and direct on-going Occupational Therapy programmes to patients provided by reablement therapy workers as required.
Contribute to the formulation and evaluation of individual care plans, working closely with patients, their families and carers and other members of the MDT.
Communicate in an appropriate and professional manner, using the systems in place and in accordance with organisational policies and procedures
The Ideal Candidate
Essential criteria:
HCPC Registered Physiotherapist or Occupational Therapist
Evidence of continuing professional development
Mentorship/teaching qualification (within first 2 years of post)
Educated to diploma level
Valid UK driving licence and access to a vehicle for work purposes
