Overview
We are seeking a compassionate and dedicated RN or LPN to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care and will be responsible for assisting in the delivery of nursing services in various healthcare settings. This role requires a blend of clinical skills, patient interaction, and administrative duties to ensure the well-being of our patients.
Responsibilities
Conduct patient assessments to gather vital information regarding health status and medical history.
Administer medications and treatments as prescribed by physicians.
Assist DON/ADON in daily duties
Complete MDS assessments as delegated
Provide care in home care settings, ensuring comfort and safety for patients.
Support medical-surgical procedures and assist with coding for medical records.
Collaborate with healthcare professionals to develop and implement individualized care plans.
Engage with patients experiencing behavioral health issues, providing empathetic support.
Maintain equipment related to Durable Medical Equipment (DME) as needed.
Document all nursing interventions and patient responses accurately in medical records.
Qualifications
Current state licensure as a Licensed Practical Nurse (LPN) OR RN
Proven experience in acute care or home care settings is preferred.
Strong skills in patient assessment and medical documentation practices.
Familiarity with specimen collection processing, IV insertion, and coding procedures is advantageous.
Ability to work collaboratively within a multidisciplinary team while maintaining a high level of professionalism.
Excellent communication skills, both verbal and written, with a focus on patient interaction.
