Job Summary
The Patient Care Manager (PCM) is responsible for ensuring that patient care is coordinated and managed appropriately in coordination with the Director of Patient Care Services. The PCM is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. The PCM implements and evaluates goals and objectives for hospice services that meet and promote standards of quality and contribute to the total organization and philosophy.

Duties

Available on premises or accessible by telecommunications during operating hours.
Coordinates and oversees all direct and indirect patients’ services provided by the clinical organization personnel as assigned by leadership based on organizational needs.
Provides guidance and oversight for patient care with all clinical field staff.
Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
Provides help in assessment, planning implementation and evaluation of patient and family /caregiver care to all clinical personnel as indicated.
Assists DPCS and/or PCD in managing clinical teams and planning with oversight of DPCS and/or PCD.
Assures in collaboration with compliance and quality department proper maintenance of clinical records in compliance with local, state and federal laws and regulations.
Collaborate with Office Administrative staff to ensure the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services.
Collaborates with Clinical Leadership, Quality Leadership and Human Resources for the development, implementation and evaluation of new hire orientation and ongoing training.
Assists Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.
Complies with accepted professional standards and principles.
Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs. Participates in public relations and community activities that promote the organization's role as an effective member of the hospice and health care delivery system. Promotes customer service orientation to all hospice personnel.
Participates actively in quality assessment performance improvement teams and activities.
Performs other duties and activities as delegated by the Director of Patient Care Services and / or the Administrator.
A requirement and expectation of this clinical leadership role is the provision of direct bedside patient care as needed or as necessitated by Quality-of-Care standards, patient safety/well-being needs, customer experience/expectations, business needs, or in times of staffing challenges. This may include, but not be limited to, new Admissions, Case Management, PRN visits, higher level of care assessment or care, and may be during business hours or after-hours, including weekends.

Requirements

Registered nurse with current licensure to practice professional nursing in the state.
Graduate of NLN accredited school of nursing with a BSN degree is preferred or a minimum of one (1) year experience as a professional nurse in Hospice/Home Health setting within the last two years.
Proven ability to work within an interdisciplinary setting demonstrated positive and effective collaboration with engaged positive customer service and patient care.
Comply with accepted professional standards and practice.
Have excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.
Understands principles of pain/symptom management.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Complies and maintains current CPR certification.
Demonstrates excellent observation, verbal and written communication skills.

Job Type: Full-time

Pay: $135,000.00 - $150,000.00 per year
