As a Patient Care Coordinator, you will play a crucial role in ensuring that our patients receive the highest quality care. You will be responsible for coordinating all aspects of patient care, including scheduling appointments, maintaining accurate patient records, and providing support to patients and their families. Responsibilities:
• Coordinate and manage all aspects of patient care, ensuring that patient needs are met efficiently and effectively at Pristyn Care.
• Schedule patient appointments, follow-ups, and procedures, ensuring smooth operations and minimal wait times.
• Maintain and update accurate patient records, ensuring all information is up to date and compliant with healthcare regulations.
• Provide support and assistance to patients and their families, addressing concerns, answering questions, and offering guidance.
• Assist medical staff by preparing patients for examinations, explaining procedures, and ensuring that all necessary information is available.
• Communicate effectively with patients, families, and healthcare providers to ensure a seamless healthcare experience at Pristyn Care.
• Monitor patient progress and report any significant changes to the relevant medical staff.
• Ensure patient confidentiality is maintained at all times, following Pristyn Care's standards and healthcare regulations.
• Coordinate with insurance providers to verify patient coverage and manage any billing or claims-related issues.
• Participate in quality improvement initiatives to enhance patient care services at Pristyn Care.
Requirements:
• Bachelor's degree in Healthcare Administration, Nursing, or a related field.
• At least two years of experience in a healthcare setting, preferably in a patient care coordination role at Pristyn Care.
• Strong knowledge of medical terminology, procedures, and coding.
• Excellent communication and interpersonal skills, with the ability to empathize with patients and families.
• Proficiency in Microsoft Office Suite and healthcare software.
• Ability to multitask, prioritize tasks, and manage time effectively in a fast-paced environment.
• Strong organizational skills with attention to detail.
