About the role

The Clinical Nurse Specialist – Neurofibromatosis Clinic plays a pivotal role in delivering expert, patient-centred care for adults with neurofibromatosis at The Royal Melbourne Hospital. Working within a multidisciplinary team, you will provide advanced clinical assessment, care coordination, patient education, and ongoing support for patients and their families as they navigate complex and often lifelong healthcare needs.

As a key member of the service, you will act as a central point of contact for patients, liaising with neurosurgeons, oncologists, nursing, allied health, primary care providers, and external treatment services to ensure seamless, coordinated care. You will support multidisciplinary care planning, facilitate referrals and follow-up, contribute to outpatient clinics, and help ensure patients receive timely access to the services and treatments they need.

Key Accountabilities

Coordinate and communicate patient care and treatment plans with medical, nursing, allied health and external providers.
Participate in multidisciplinary team meetings, document outcomes and coordinate follow-up actions.
Act as the primary contact for patients, families and carers, providing education, support and care navigation throughout the treatment journey.
Coordinate referrals, discharge planning, appointments and ongoing care to ensure seamless transitions across services.
Liaise with research teams and external treatment providers to support timely, coordinated patient care. Maintain patient education resources and provide education and clinical support to staff.
Contribute to the development of shared models of care with primary care providers for appropriate patient groups.

About You

Current AHPRA registration as a Registered Nurse.
Strong clinical reasoning, prioritisation and communication skills.
Ability to work collaboratively within a multidisciplinary team.
