Inpatient Care Coordinator

Position Summary
TheIn-patient Care Coordinator are core members of Lawndale Christian Health Center's (LCHC) care teams, defined as a small group of clinical and non-clinical professionals working together to manage the health of LCHC patients. The In-patient Care Coordinatorare responsible for leading disease management and prevention efforts, referral coordination, and
patient follow-up for hospitalization and emergency department visits.
The In-patient Care Coordinator will be primarily stationed at Mt. Sinai Hospital (MSH) and St. Anthony Hospital (SAH), partner hospitals where the majority of LCHC patients are admitted. The In-patient care Coordinator is LCHC's representative there and plays a critical role in ensuring the smooth transition of care between the hospital and outpatient setting by scheduling and tracking appropriate follow-up visits, coordinating medication reconciliation prior to hospital discharge, and providing patient education to reduce unnecessary and costly hospital visits. The In-patient Care Coordinator must be proactive, well-organized, and function well in a fast-paced clinic environment.
Qualifications
• Demonstrates excellent communication skills.
• Displays independence and self-motivation; ability to initiate and achieve tasks without extensive supervision and feedback.
• Strong ability to multi-task and work well in a fast-paced environment.
• Requires experience and demonstrated abilities for working in a multi-cultural setting.
• Flexibility to work evenings and/or weekends if necessary.Proficiency in excel and basic statistical analysis.
• Embraces and personifies the mission of the Lawndale Christian Health Center, providing for the physical, emotional, and spiritual needs of staff members and patients.
Required Knowledge, Experience, or Licensure/Registration
• B.S./B.A. in social/health sciences, nursing, or related degree required.
• Fluency in Spanish required.
