THEMA Health Services is seeking a motivated, results-oriented Hospice Transitional Care Specialist to support our mission of delivering compassionate, high-quality end-of-life care across Arizona. In this sales-driven role, you will focus on building strong referral partnerships and facilitating smooth care transitions for patients moving from hospitals, skilled nursing facilities (SNFs), assisted living facilities (ALFs), and other sources into THEMA's hospice services. You will educate healthcare professionals, patients, and families on the benefits of hospice care—emphasizing comfort, dignity, symptom management, and support from our interdisciplinary team—while driving referral growth and admissions in the competitive Scottsdale/East Valley region. Performance is measured by referral volume, conversion rates, and relationship strength within the local healthcare community.
We are seeking a dynamic and compassionate Hospice Transitional Care Specialist to join our dedicated team. In this vital role, you will facilitate seamless transitions for patients moving from hospital or acute care settings back into hospice or home-based care. Your efforts will ensure patients receive timely support, education, and coordination to improve their comfort and well-being during this critical phase of life.
Act as the primary sales liaison and relationship manager between key referral sources (hospitals such as HonorHealth and Banner facilities, physicians, discharge planners, case managers, SNFs, ALFs, and other providers), patients/families, and THEMA's clinical hospice team to ensure seamless, compassionate transitions.
Conduct proactive daily sales outreach, in-person facility visits, educational presentations, and follow-up to cultivate and expand referral networks throughout the Scottsdale/East Valley.
Develop and execute targeted strategies to increase hospice referrals and market share for THEMA in the region.
Educate referral partners, patients, and caregivers on hospice eligibility criteria, THEMA's comprehensive services (including pain and symptom management, emotional/spiritual support, 24/7 availability, and interdisciplinary care), and the streamlined admission process.
Coordinate timely hospice evaluations, informational visits, and admissions; collect required clinical documentation and collaborate with discharge teams to support quick, efficient enrollments while minimizing hospital readmissions.
Advocate for appropriate hospice placement during patient transitions, ensuring alignment with Medicare guidelines and THEMA's commitment to dignity and quality of life.
Maintain accurate tracking of referral activity, sales pipeline, admission outcomes, and territory metrics using CRM systems.
Participate in healthcare networking events, community outreach, and industry activities to raise awareness of THEMA's hospice services and generate new leads.
Stay current on hospice regulations (Medicare Conditions of Participation, Arizona licensing), competitive dynamics in the East Valley, and best practices in transitional care.
Partner closely with THEMA's internal clinical, admissions, and leadership teams to overcome barriers and deliver exceptional experiences for patients and families.
Bachelor's degree in healthcare, business, marketing, communications, or related field preferred (equivalent experience will be considered).
At least 2+ years of demonstrated success in sales, marketing, liaison, or business development roles within hospice, home health, palliative care, hospital discharge planning, or post-acute settings.
Solid knowledge of hospice care principles, the Medicare hospice benefit, transitional care processes, and typical referral pathways.
Proven ability to meet or exceed sales/referral targets and foster long-term professional relationships.
Outstanding interpersonal, presentation, and communication skills; capable of building trust with families during difficult times while maintaining a professional, goal-oriented sales approach.
Highly self-motivated, organized, and comfortable working independently in a field-based environment with regular travel (reliable vehicle, valid Arizona driver's license, and current auto insurance required).
Proficiency in CRM software, Microsoft Office Suite, and virtual collaboration tools.
Genuine compassion and passion for supporting patients and families through end-of-life care.
Preferred Skills & Experience
Prior experience as a hospice liaison, community liaison, transitional care coordinator, or business development professional in Arizona markets.
Established connections or familiarity with the Scottsdale/East Valley healthcare network (e.g., HonorHealth, Banner Health, local SNFs/ALFs).
Clinical background (e.g., RN, LVN, or social worker) is advantageous but not required for this sales-focused position.
