Fraserside Community Services Society invites applications for the following position:

Job Title: HOME SHARE SUPPORT WORKER

Program: HOME SHARE PROGRAM

Classification: Standard Grid Level 9 – Unique

Rate of Pay: $27.83 per hour and increments in accordance with the terms of the Collective Agreement

JOB SUMMARY:

The Home Share support worker assists the Home Share Coordinator in screening applications to become Home Share providers and selecting home share providers, as per CLBC home share standards. The Home share support worker provides administrative support to the Home Share coordinator by documenting care plan for the clients, monitoring the progress of clients in the caregiver’s home and reporting any challenges to the Home Share Coordinator.

QUALIFICATIONS:

Minimum Diploma in a related human service field and one year recent experience in day services and developing community opportunities for people with a developmental disability or an equivalent combination of education, training and experience
· Working knowledge of MS Office; familiarity with an electronic case management system is preferred.

Demonstrated skills in teamwork, individual and group planning, training and support
Ability to communicate effectively with clients, support networks and the community in general
· Ability to work independently and in isolated settings, dealing with emergencies as they occur

Strong attention to detail and ability to self-direct
Demonstrated ability and commitment to working effectively with diverse populations
· Demonstrated skills in the area of planning, decision making, coaching and building positive working relationships

· Familiarity with positive behavioral support techniques and support the caregivers in practicing these techniques

Valid First Aid/CPR and Nonviolent Crisis Intervention certification
Class 5 Driver’s License; Reliable personal vehicle
HOURS OF WORK: 30 hrs./week; Start and finish times may vary and flexibility with hours of work is required to meet client and program needs as determined by the Program Coordinator.
