Advanced Home Health and Hospice of Las Vegas is looking for an RN Case Manager to join our growing home health team on a PRN or part-time basis! Case Management will primarily take place in North and Northwest Las Vegas. Candidates will be friendly, positive and focused on excellence! Advanced Home Health is an agency where the focus is on delivering 5-star services and exceptional patient outcomes.
Job Responsibilities:
• Perform admission, discharge, and routine home health visits.
• Coordinate & communicate patient care plans with interdisciplinary team.
• Document all phone calls and visits using our EMR.
