Job Description
Join our dedicated healthcare team as a Home Health Registered Nurse, where you will play a vital role in delivering personalized, high-quality nursing care directly to patients in their homes. This dynamic position offers the opportunity to assess patient needs, develop tailored care plans, and coordinate services to promote recovery, independence, and well-being. As a key member of our home health team, you will utilize your extensive clinical expertise to ensure safe, effective, and compassionate care delivery in a variety of patient environments.
Responsibilities
Conduct comprehensive patient assessments including vital signs, physiological evaluations, and medical history reviews to determine care needs.
Develop and implement individualized care plans in collaboration with physicians, patients, and families to optimize health outcomes.
Manage patient care activities such as medication administration, wound care, IV therapy, and post-acute recovery support.
Monitor patient progress through regular evaluations and adjust care strategies accordingly to ensure safety and effectiveness.
Document all patient interactions accurately using electronic health records (EHR) management systems while maintaining compliance with HIPAA regulations.
Coordinate discharge planning and facilitate communication among healthcare providers to ensure seamless transitions across settings.
Educate patients and caregivers on disease management, medication adherence, and health maintenance strategies to foster independence.
Experience
Proven clinical experience in Acute or home care settings
Experience with infectious disease care, geriatrics, behavioral health and ability to serve diverse patient populations effectively.
Familiarity with medical-surgical nursing, post-acute care, outpatient clinics, and managed care models is preferred.
Strong knowledge of health regulation policies, medical terminology, physiology principles, and utilization management practices ensures compliance and quality standards are met.
Prior experience managing patients as a nurse case manager or in primary care settings supports comprehensive case management skills.
Proficiency in electronic health records (EHR),( AXXESS software preferred) medical records documentation, diagnostic evaluation processes, and discharge planning is essential for success in this role.
