Dementia Centre Advisor
Job Description
A rewarding opportunity for a passionate Dementia professional to work with our team at HammondCare on a Permanent Full-Time basis.
The role focuses on improving the lives of individuals with dementia across HammondCare services by building staff capacity to deliver best practice care.
We are looking for someone to support staff at all levels in implementing HammondCare's Model of Care and Case Management framework, and contribute to knowledge generation on best practice dementia care.
Responsibilities:
Understand and engage with HammondCare Mission in Action, Model of Care, Case Management Framework and practice guidelines, policies and procedures
Provide dementia specific expert advisory services within HammondCare services through participation in regular clinical review meetings and on-the-ground support
Build staff capacity through communication, education, mentoring, and coaching in practice
Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements
Requirements:
A minimum of 3 years' experience in the Australian Aged Care Industry
Registered Nurse or Allied Health qualification
Demonstrated skills in teaching, mentoring and coaching others
Knowledge and skills in relation to adult learners and development of learning resources
Ability to prioritize multiple tasks and meet deadlines
Strong interpersonal and communications skills both written and verbal
