Job Description

The Department of Aging and Adult Services - Office of the Public Guardian is recruiting for a Community Wellness and Housing Coordinator*. This position will supervise operations and projects including Pacific Village, while serving as a liaison with County departments and divisions. Pacific Village is a campus-style program that provides supportive housing along with on-site medical care, mental health services, and substance use treatment for individuals experiencing homelessness. It uses a wraparound services approach to help residents stabilize, recover, and transition toward long-term independence. A current vacancy exists in San Bernardino.

Duties may include, but are not limited to, the following:

Supervise support staff; assign and review work; prepare and sign work performance evaluations; participate in hiring and disciplinary actions.
Develop project plans and timelines; researches, analyzes and prepares recommendations regarding projects to management; establishes and maintains a system for tracking project activities, progress, and outcomes; monitors project progress and provides technical and administrative support.
Gather and analyze information pertaining to project needs, including overall project evaluation; develop and monitor data collection systems.
Provide input on operational policies and procedures as it relates to the projects.
Maintain a thorough knowledge of department and county policies and procedures, and state and federal laws that may affect the department.
Research, compile and organizes information for use by management and identifies both direct and indirect impacts of recommendations made.
Provide input on project related budget requests, including equipment and material forecasts for submission to management. Prepare a variety of reports, records, correspondence and other documents.
Prepare written reports, executive summaries, and presentations for leadership that clearly communicate trends, risks, successes, and recommendations based on data analysis.
Lead coordination with behavioral health, housing, homeless services, and other community partners to ensure integrated service delivery and resolve operational barriers impacting unhoused individuals and permanent supportive housing, recuperative care and or substance use treatment.
Oversee critical incident response and ensure appropriate follow-up, documentation, and interagency communication to support resident safety and program compliance.
Monitor program performance metrics related to mental health, housing stability, and service utilization; recommend system improvements to enhance resident outcomes and program efficiency.
Provides vacation and temporary relief as required.

CONDITIONS OF EMPLOYMENT

Travel: Travel throughout the County is required and employees will be required to make provisions for such transportation. A valid California Class C driver license and proof of automobile liability insurance must be produced and maintained for the individual providing the transportation.

Minimum Requirements


Applicants may qualify under Option A or Option B, but must meet both of the education and experience requirements within that option:

OPTION A

EDUCATION: Thirty (30) semester (45 quarter units) of completed college coursework from an accredited college or university half of which must be upper division, business/public administration, behavioral/social science, psychology, sociology, social services, human services, education or closely related field.
-AND-
EXPERIENCE: Three (3) years of full time equivalent project management experience in which the primary responsibility includes oversight and coordination of projects (e.g. facilitate and monitor program objectives and budget, initiate, develop and write grants, research and data analytics for project needs including overall project evaluation) in a Human Services or related field.
