Ensuring the safe and effective use of medications. This includes reviewing patient prescriptions for accuracy, appropriateness, and potential drug interactions.
Conducting medication reviews and assessments to identify potential problems with drug therapies, such as side effects, contraindications, or duplications.
Collaborating with doctors, nurses, and other healthcare providers to optimize pharmacological treatment plans for patients, offering evidence-based recommendations.
Educating patients about their medications, including how to take them, potential side effects, and the importance of adherence to prescribed regimens.
Monitoring patients' responses to medications, tracking outcomes, and making adjustments to treatment plans as necessary.
Offering comprehensive medication reviews to improve health outcomes, particularly for patients with chronic conditions or complex medication regimens.
Identifying, documenting, and reporting any adverse drug reactions to ensure patient safety and contribute to safety data.
Providing education to healthcare professionals and patients on the proper use of medications and the latest therapeutic guidelines.
Participating in or supporting clinical trials and research to advance pharmacological knowledge and improve patient care practices.
