Reporting to the Clinical Manager, the Clinical Case Coordinator is a member of the interprofessional clinical team. This role provides comprehensive case management services to youth admitted to SAYC throughout their stay. The Clinical Case Coordinator supports youth during admission and identifies and coordinates internal and external resources for youth and their families. In addition, the Clinical Case Coordinator collaborates with the Intake Committee to coordinate the intake process.
The hourly rates for this position are: Step 1: $ 35.43 - Step 2: $37.19 - Step 3: $39.06
Duties & Responsibilities
Provide case management to coordinate and support services for youth and their families during admission at SAYC
Identify, develop, and coordinate appropriate discharge plans in collaboration with internal and external stakeholders
Maintain clear, timely, and accurate verbal and written communication, including clinical documentation related to youth care and treatment
Coordinate and facilitate meetings and appointments as required
Collaborate with other Clinical Case Coordinators on a rotating basis to support the intake process, including: Initial contacts and assessments, Triage and application review , Screenings and scoring of standardized instruments, Recommendations regarding admission priorities
Qualifications
Bachelor of Social Work (BSW) required (Candidates with a Social Service Worker diploma and 5+ years of relevant experience may be considered)
Eligible for registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
Strong written and verbal communication skills
1–5 years of experience in a related setting
