Certified Case Management Specialist
Job Description
Key Responsibilities:
• Actively manage resource utilization to ensure optimal patient outcomes during rehabilitation.
• Coordinate and advocate for patients throughout their hospital stay, from admission to post-discharge.
• Facilitate effective communication regarding patient care and drive collaboration on the interdisciplinary plan of care.
• Establish and monitor the implementation of discharge plans, addressing patient psychosocial and support systems needs.
• Oversee services coordination and manage critical issues in admission, discharge, and interdisciplinary communication.
• Provide patient and family education, support payor relations, and manage fiscal responsibilities.
• Conduct ongoing utilization reviews and act as a liaison to payors to ensure cost-effective treatments.
Qualifications:
• Must have qualifications to independently complete assessments in your discipline (e.g., RN, SW, OT, PT, ST, or Rehabilitation Counseling).
• Hold an active license if required by your state.
• Current CCM or ACM certification is mandatory.
• For Nurses: Minimum of an Associate Degree in Nursing is required, with RN licensure; a BSN is preferred. Diplomates are acceptable where licensure requires it.
• For other eligible health care professionals: a minimum of a bachelor's degree is required, with a graduate degree preferred.
• At least 2 years of rehabilitation experience is preferred.
Physical Requirements:
• Good visual acuity and effective communication skills are essential.
• Ability to lift, lower, push, pull, and retrieve objects weighing at least 30 pounds.
Work Environment: This position is based in a temperature-controlled, smoke-free environment with occasional outdoor exposure. Be prepared for exposure to blood and body fluids, and the ability to work under occasional stressful conditions.

• Meet established attendance standards and dress code policies.
• Flexibility to work evenings, weekends, or holidays as necessary to meet deadlines.
• May be required to work during disaster situations and assist beyond regular hours if needed.
