Job Description
Hours: 15 hours per week to start, Monday to Friday, approximately 3 hours per day
Start Time: Morning shifts, generally around 8:00am or 8:30am, with some flexibility to allow for participant meetings
Growth Opportunity: Hours may increase as the business continues to grow
Location: Work from home with some travel to participant and staff face-to-face meetings

We are currently seeking a Care Designer to join our team in a permanent part-time role. This position would suit someone who is highly organised, confident communicating with participants and families, and able to balance care planning, rostering, staffing and general enquiries.
This role will commence at 15 hours per week, working 3 hours per day, Monday to Friday, with the potential to increase as the company grows.
About the Role
The Care Designer will support the intake, onboarding, care planning and rostering process for new and existing participants. You will work closely with participants, families, support coordinators and support workers to help ensure supports are planned, staffed and delivered smoothly.
This role will be completed under the oversight of the Business Manager, with clear processes, templates and guidance provided.
The position will include a mix of office-based administration, phone and email communication, rostering, and face-to-face care planning meetings with participants across the Gold Coast.
Key Responsibilities
Your duties will include:
Client Intake and Onboarding
- Managing new participant leads and enquiries
- Following up email and phone enquiries
- Arranging and attending face-to-face care planning meetings with participants across the Gold Coast
- Gathering relevant information about participant goals, routines, preferences, risks and support needs
- Preparing care plan reports and onboarding documentation under the oversight of the Business Manager
- Assisting with the design of supports, including community access, in-home support and respite arrangements
