The Role
We are seeking a Care Coordinator to join our team in a part-time capacity (32 hours per week over four days). In this role you will play a vital role in delivering high-quality services through assessment, case management, and service monitoring for eligible clients, aligned with McLean Care’s values and policies. Reporting to the Clinical Care Manager of Home and Community Care Services NSW, you will need:

A Certificate III or IV in Aged Care (or equivalent) is required for this role
The ability to balance client expectations with available resources, effectively communicating these to clients.
Develop and review individualised care plans based on client assessments and preferences.
Excellent communication, interpersonal, and negotiation abilities.
Maintain accurate records and documentation in line with organisational policies and government funded guidelines
A collaborative approach to working as part of a team.
Knowledge of Commonwealth Support Programme (CHSP) is highly valued.
Demonstrated experience in assessment, referral, and coordination of human services.
An unrestricted driver’s license.
This role requires travel to client’s home however office-based tasks can be completed remotely (WFH).
