Job Title: Bank B5 Community Staff Nurse - INW
NHS AfC: Band 5, Nursing - Central London Community Healthcare NHS Trust
Location: Central London, Greater London
The Community Matrons Team provides support to housebound patients with multiple health and social care needs to prevent unnecessary hospital admission and achieve better health outcomes.
This role is a Monday-to-Friday, +am-5pm position across Westminster.
Key Responsibilities:
Monitor patients with complex conditions and identify potential deterioration.
Conduct vital signs observations, including urinalysis, weight, and MUST scores.
Strong communication, administration, and documentation skills are essential for this role.
Familiarity with S1 systems is required.
Ability to work independently and remotely from colleagues is necessary.
Traveling to various locations in Westminster may be required, which may involve walking with equipment in a trolley bag.

Requirements:
Registered nurse or nurse associate qualification.
Experience in community health is essential.
A strong understanding of patient monitoring and identification of potential issues.
Effective communication and administrative skills are necessary.
Familiarity with S1 systems.
Ability to work independently and as part of a team.
Estimated salary range: £31,250 - £37,8+0 per annum.
