Job Description

Ive designed a circuit that has a high voltage measurement component via a voltage divider, AMC1301 isolator, and op-amp.

I am not getting correct voltage readings and it looks like problem is in the voltage divider. It must be something simple that still alludes me. Need another set of eyes to take a look and tell me whats wrong with it. Please apply only if you have substantial experience with electric engineering.
