The Alzheimer Day Program Worker will be required to work Monday to Friday from 8:30 AM to 4:30 PM.

Position Summary

Reporting to the Manager, Programs and Services, the Alzheimer Day Program, Program Worker is responsible for the development and implementation of program activities for participants of the Alzheimer Day Program and the St. James Town, Day Program (a partnership program with Progress Place).

Duties and Responsibilities
Assist in planning, implementing, monitoring and evaluating program activities.
Observe and report changes in clients’ physical, emotional, mental health and/or cognitive functioning.
Assist clients with activities of daily living, such as toileting, transferring or eating.
Provide caregiver support and resources.
In consultation with the program coordinators provide information referral to clients caregivers about other support services and community resources
Conducting client follow-up.
Maintain accurate client records and statistics.
Provide support and accompaniment to clients to or from program on agency vehicles.
Support partnership program through activity development and implementation
Participate in meetings and other relevant tasks as required.
Maintain a clean and safe environment for participants, caregivers, volunteers and staff.
Work collaboratively with program staff, supervisor, program manager, partners and other Dixon Hall staff and volunteers.
Actively participate in department and agency meetings and events.
Work under the guidance of the agency and program policies and procedures, including the mission, vision and values of Dixon Hall.
Meet regularly with supervisor to discuss individual, team and program effectiveness.
Undertake other tasks and responsibilities as assigned, within the purview of the job description.
Other duties as required.

Qualifications
2-3 years experience working with seniors who are frail and or individuals with Alzheimers Disease or other cognitive impairments.
Knowledge and experience in providing social and recreational programming for seniors and adults with disabilities.
Excellent organizational, communication and interpersonal skills.
Good working knowledge of Outlook, Word and other MS Office applications.
Ability to exercise sound judgment.
Ability to work collaboratively with team members and others.
Fluency in a second language reflective of the Downtown East Toronto area an asset.
Relevant College diploma/University degree (all foreign diplomas recognized).
Must be qualified to work in Canada legally for the employer.
Able to provide a Vulnerable Sector Screening check with a ‘Clear’ or satisfactory result that meets the operational requirements of the Agency.
Must have up-to-date vaccinations against COVID-19 with a vaccine that has been approved for use in Canada.
