Transition Officer

We are seeking an experienced Transition Officer to work with us at Brisbane Youth Education and Training Centre for a temporary full-time position until 30 June 2025.
About The Role
The Transition Officer is responsible for meeting placement and outcome targets by successfully engaging and supporting students to transition into education and employment and sustaining their placement post-detention. This role involves collaborating with a range of stakeholders, including students and carers, Queensland Government partner agencies, and agencies from the broader service system.
Key Responsibilities
Plan and facilitate the transition to employment, training, and/or educational settings for students in youth detention in collaboration with students and relevant stakeholders.
Establish rapport and maintain resilient relationships with students to motivate their engagement in learning, as appropriate to student's individual circumstances, to enable successful transition plan outcomes.
Be accountable for assigned cases to progress transition plans, including undertaking appropriate case management, advocacy, and support, prioritising daily caseload, communicating with transition partners and students, and implementing agreed approaches.
Communicate expectations of the transition program with students and stakeholders as required to develop their understanding of career and learning pathways and promote the importance of continued education for students while in detention.

We are looking for someone who can implement strategies and programs to improve the transition outcomes of young people who may have had varied schooling experiences and a proven capacity to work effectively with them to address their future learning, training, and employment pathways.
