Duties

Engage in case management activities, coordinating with healthcare providers to meet the needs of clients.
Implement behaviour management strategies to support individuals with specific needs.
Maintain accurate records of client care and progress, using appropriate medical terminology.
Offer companionship and emotional support, fostering a positive environment for clients.
Assist clients with daily living activities, including meal preparation, community based support and mobility support.
Collaborate with multidisciplinary teams to develop and implement personalised care plans.
Participate in training and professional development opportunities to enhance skills in senior care and hospice care.
Requirements
Previous experience in the care sector is highly desirable.
Familiarity with case management practices and medical terminology is advantageous.
Ability to demonstrate compassion, patience, and empathy towards clients.
Strong communication skills, both verbal and written, for effective interaction with clients and team members.
A commitment to maintaining client confidentiality and adhering to health and safety regulations.
