The Position:
We are seeking a Float Social Worker to support our Inpatient Rehab and Complex Continuing Care (CCC) services in delivering exceptional rehabilitation services to children and youth.
Inpatient services at Holland Bloorview are subdivided into three distinct areas - Brain Injury Rehab (BIRT), Specialized Orthopedic and Developmental Rehab (SODR), and Complex Continuing Care (CCC). This is a regular full-time (1.0 FTE or 37.5 hours/week) position.
Key Responsibilities
Conducts strengths-based biopsychosocial assessments for clients and families of diverse backgrounds
In collaboration with the interdisciplinary team, support clients and families to enhance coping and advocacy skills
Identifies need for social work intervention through case screening and case finding methods
Supports and respects the client/family’s rights to make decisions based on their own values, beliefs and experiences
Provides counselling to clients utilizing evidence-based techniques based on client’s presenting mental health concerns
Initiates and coordinates referrals and support system navigation with regards to funding, community resources and other healthcare services to enable a successful discharge
Advocates for client and family needs for service within Holland Bloorview and the community
Bases practice on established Holland Bloorview policies, procedures, evidence and best practice
Documents accurate workload statistics
Attends and participates in program and professional staff meetings
Participates in quality improvement initiatives, contributes to the academic agenda through education or research and supports the maintenance of a safe and healthy work environment that advances a culture of client/patient safety through work and daily practices
Adheres to Holland Bloorview’s Documentation Policy
Communicates in a timely and effective manner
Contributes to the development of program/service activities
Ensures the maintenance of the OCSWSSW Standards of Practice and Code of Ethics for Social Workers
Identifies learning needs and establishes annual goals
Remains current with professional literature and best practice guidelines
Shares new knowledge and best practices with colleagues, staff, students and the community
Qualifications
MSW Degree from an accredited University, Faculty of Social Work
Current member of the Ontario College of Social Workers and Social Service Workers
Current member of the Ontario Association of Social Workers preferred
Paediatric social work or health care experience preferred
Previous experience working with youth with disabilities and their families is an asset
Familiarity with resources in the community that support this population
Demonstrated knowledge of common psychotherapy frameworks such as ACT, ACBT, DBT and Solution-Focused Therapy and an ability to apply them in session as appropriate
Experience working in an interdisciplinary setting preferred
Works effectively as a member of the interdisciplinary team
Demonstrates ability to manage and respond to crisis calmly and effectively
Demonstrates ability to organize time and meet deadlines
Demonstrates initiative and self direction
