Role: RN Care Coordinator - DTLA
Reports to: Director, Health and Wellness
Location: The Village, 527 Crocker Street, Los Angeles, CA +0013
Schedule: Monday-Friday, 8:30am-5:00pm
Status: Full-time; Exempt; Non-Management

Summary
The People Concern, in partnership with community and hospital organizations, provides on-site medical management and healthcare coordination for individuals experiencing homelessness. The Health Services Department delivers direct medical care, education, and coordination of healthcare services in alignment with the RN scope of practice.
The RN Care Coordinator collaborates with interdisciplinary teams in interim housing and community settings to connect participants—both housed and unsheltered—to appropriate medical resources. The role supports improved health outcomes and reduced reliance on emergency and inpatient services through coordinated, trauma-informed, and participant-centered care.
Duties and Responsibilities
Utilize a harm reduction and trauma-informed approach to provide care for individuals with acute and chronic medical conditions.
Provide outreach services to increase enrollment and engagement for ECM program.
Support the Enhanced Care Management (ECM) team in service delivery and compliance with applicable regulations.
Serve as a clinical liaison between ECM programs, healthcare providers, and community resources.
Supervise and mentor program staff, including LVNs and Enhanced Care Managers.
Assist with escalated clinical cases and provide support during difficult situations.
Coordinate and facilitate referrals, screenings, insurance verifications, and care planning.
Act as a clinical resource to staff and facilitate connections to primary and behavioral healthcare.
Monitor client well-being, conduct wellness checks, and contribute to discharge planning.
Accompany clients to medical appointments and advocate for their needs.
Deliver nursing services including triage, wound care, medication reconciliation, and general care within RN scope.
Educate clients, staff, and community partners through workshops and health presentations.
Promote client independence through SMART goal setting and self-care education.
Ensure quality of care through outcome evaluation, adjustments to care, and adherence to standards.
Document care activities accurately and timely in accordance with program guidelines.
Maintain HIPAA compliance and privacy practices.
Contribute to the development of medical policies and protocols.
Pursue ongoing professional development and maintain licensure.
Foster teamwork and cooperation among care teams.
Ensure a clean and safe work environment.
Perform other duties as assigned.
Qualifications
Nursing degree from an accredited nursing program.
Current California Registered Nursing license.
Minimum one year of nursing experience post-licensure.
Team-oriented with excellent communication and documentation skills.
Experience with diverse populations including LGBTQ+ and individuals with mental illness or substance use disorders.
Passion for serving underserved and vulnerable populations.
Proficiency in Microsoft Office applications.
Valid California Driver's License, good driving record, and valid insurance.
Preferred Qualifications
Experience working with homeless or underserved populations.
Bilingual or multilingual, especially in Spanish.
Familiarity with Los Angeles County health and social services.
Work Environment
Office-based with occasional field work.
Regularly requires sitting, standing, bending, and lifting up to 35 pounds.
May be exposed to varying environmental elements such as weather and noise.
Busy and dynamic workplace requiring flexibility and adaptability.
