Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that patients receive the appropriate services and resources necessary for their health and well-being. You will work closely with healthcare providers, patients, and their families to develop individualized care plans and facilitate effective communication among all parties involved.

Duties

Assess patient needs and develop comprehensive care plans tailored to individual circumstances.
Coordinate with multidisciplinary teams to ensure seamless transitions of care across various settings.
Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
Educate patients and families about health conditions, treatment options, and available resources.
Advocate for patients within the healthcare system to ensure they receive appropriate services.
Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
Collaborate with insurance providers for utilization management and managed care processes.
Participate in quality improvement initiatives to enhance patient outcomes.
Skills

Strong understanding of hospital medicine, critical care, and pediatrics.
Proficient in medical terminology relevant to patient care.
Experience working in Level I or Level II Trauma Centers is highly desirable.
Knowledge of utilization management principles and managed care practices.
Excellent communication skills for effective interaction with patients, families, and healthcare professionals.
Ability to work collaboratively in a fast-paced environment while managing multiple priorities.
Strong problem-solving skills to address complex patient needs effectively.

Job Type: Full-time

Pay: $75,000.00 - $85,000.00 per year
