**About the Role**

At Health New Zealand - Te Whatu Ora Wairarapa, we are seeking a highly skilled Registered or Enrolled Nurse to join our dynamic Outpatients team. As a key member of our Planned Services team, you will play a vital role in providing timely, well-coordinated services to patients from referral through to discharge.

**Key Responsibilities**

As a Registered or Enrolled Nurse, your primary responsibilities will include:

Providing exceptional patient care and communication skills
Working collaboratively with the healthcare team to achieve optimal patient outcomes
Utilizing case management concepts to ensure seamless patient transitions
**Requirements**

To be successful in this role, you must possess:

A current annual practising certificate
Proven ability to work confidently without supervision
Strong communication and teamwork skills
