Client Coordinator (Registered Practical Nurse)

Employment: Full-time permanent
Schedule: Days with evening shifts.
FTE: 1.0
Location: Onsite (CarePoint Health – 2695 North Sheridan Way, Suite 120, Mississauga, ON), but may also be required to work at other CarePoint Health sites within Mississauga
Starting Date: Immediate
Salary: $46,487-67-$54,691.38 and a comprehensive benefits package (including HOOPP pension)

What is this role?
CarePoint Health is seeking a client coordinator to support our unattached patient program called CarePoint Connects. This role will report into the clinical manager. The Client Coordinator will review all referral documentation and screen for eligibility, provide service navigation as needed and support patients in finding a physician that matches their needs. The Client Coordinator will work with the Physician Engagement Specialist to ensure that physician availabilities and service offerings remain up to date. The Client Coordinator will also connect patients with active health care needs to the internal clinical team for assessment and transitional supports. The candidate will also require a thorough knowledge of primary care nursing and the ability to function effectively in a team environment with physicians, nurse practitioners, registered nurses, allied health professionals and administrative staff.
This is an exciting opportunity to join a program where CarePoint Health sits at the centre of connecting thousands of Mississauga residents with access to Primary care. The successful candidates will be provided with training and will be part of a broader team supporting the CarePoint Connects initiative. This is an ideal role for a Registered Practical Nurse who has experience speaking with patients about their health needs, providing direct care, and offering service navigation and coordination.
KEY DUTIES AND RESPONSIBILITIES:
• Provide information and respond to inquiries from patients by phone and email regarding the program.
• Review referral information for completeness and complete required data elements.
• Perform telephone screening for patient referrals to assess and direct patients to the appropriate pathway within the program, ensuring relevant patient information is collected and documented.
• Communicate with the internal team and external community physician offices as needed to support successful transfer of patients for attachment.
• Coordinate with family physician practices and the Physician Engagement Team to confirm and maintain capacity information and patient intake processes.
• Work across 2-3 digital platforms and manage patient, physician and appointment information across all platforms.
• Perform data entry and data management in the digital platform used for intake. This will include setting up new patient profiles, archiving files etc.
• Assist with matching patients to family physicians based on both parties’ selection criteria.
• Learn about and stay up to date on primary care initiatives within the Mississauga region.
• Collaborate with team members at CarePoint Health and Mississauga Health to support initiatives in matching underserved patients with family physicians.
• Participate in training and ongoing education as required.
• Contribute to ongoing program development and program evaluation and implement new practices as required.
• Provide patient oriented, goal directed nursing care in accordance with the standards of care from the College of Nurses of Ontario for CarePoint Health’s programs and services.
Key Qualifications and Skills
• Registered Practical Nurse certification required.
• 2-3 years of experience in health/social services role preferably in an intake or service navigation role
• Fluency in a second language is a strong asset and preferred .
• Ability to provide genuine and respectful care to vulnerable populations.
• Knowledge of community healthcare services within Mississauga is an asset.
• Able to learn to use a variety of digital tools and platforms.
• Excellent client service skills.
• Ability to act as a liaison with other agencies and service providers.
• Strong problem-solving, decision-making and organizational skills.
• Excellent verbal and written communications skills.
• Self-directed and independent, but also able to maintain strong working relationships, problem solve and resolve conflict with inter-professional staff across the organization.
• Demonstrated excellent interpersonal and effective oral/written communication skills.
• Time management, organizational and prioritization skills to effectively address multiple tasks/duties and timelines in a fast-paced and evolving environment.
• Attention to detail in collecting and reporting patient and program information.
• Demonstrated commitment to practice based on inclusion and equity for all persons regardless of age, income, racial and ethnic origins, language, gender, sexual orientation, religion, intellectual ability, and health status.
• A growth mindset with a passion for learning and continuous improvement is required.
• Ability to uphold CarePoint Health values and be an active contributor to a positive, innovative, collaborative, and cooperative work environment.
• A satisfactory and current Criminal Background and Vulnerable Sector Screening required.
