Wage Range: $27.54 up to $31.61 (based on education and related experience)

The ASK Wellness Society is an Accredited, not-for-profit organization. Through the generous support of funders, we assist those who are marginalized and at-risk to strengthen the communities we serve (Kamloops, Merritt, and Penticton). We provide housing and support to those who are facing or at risk of homelessness and support those who may use substances, or who require mental health supports, harm reduction, or health education. Ultimately, we aim to provide the skills to re-enter the workforce.

Position Summary:

Under guidance from Lamplighter Coordinator and community health professionals, the Enhanced Care Program aims to support vulnerable unhoused individuals who frequently utilize hospital acute care due to social and housing challenges rather than medical issues. Their distinctive combination of vulnerability, care needs, behaviors, and pre-contemplative substance use highlights the necessity for tailored ongoing support to facilitate their transition into independent housing.

Typical Duties and Responsibilities:

1. Fulfill individual goals and needs as outlined in client support plans.

2. Ensure clients' health, safety, welfare, and independence, and promptly report any concerns.

3. Support the social and emotional needs of clients.

4. Assist clients with personal care and daily living activities, including bathing, grooming, skin care, toileting, and medication reminders.

5. Support client mobility and transfers, using mechanical aids as required.

6. Follow healthcare plans for tasks such as medication routines, non-sterile dressing changes, and prosthetic device management, promoting independent living.

7. Report observed changes in client health or behavior to the care team.

8. Maintain a clean and safe home environment, including tasks related to bathing, nutrition, elimination, and room maintenance, as outlined in support plans.

+. Provide nutritional support by preparing or reheating meals (breakfast and lunch), with dinner supplied by an internal meal program.

10. Participate in team meetings and client conferences as required.

11. Complete and maintain records and documentation as needed.

12. Perform other related duties as assigned.

Qualifications:

Education, Training and Experience

Grade 12, plus graduation from a recognized Health Care Assistant program; or an equivalent combination of education, training and experience. Actively registered with the BC Care Aide Community Health Worker Registry. Current valid BC Drivers’ License.

Skills and Competencies:

Commitment to working within a low-barrier work environment with persons who present with mental health, substance use, and chronic health needs
Effective verbal and written communication skills necessary for working with persons with varied social behaviours
Ability to work within an multi disciplined team environment
Ability to observe and recognize changes in clients
Ability to establish and maintain rapport with clients
Ability to organize and prioritize.
Ability to operate related equipment.
